| Performance Appraisal - Employee Pages Complete these two pages at least 2 weeks before your formal Performance Appraisal. They're designed to help you think about the goals that you'd like to work on during your next review period. The second page of this document helps you to assess how much you have accomplished over the last review period and what, if anything would help you to be more successful or satisfied on the job. Wellness Works reviews a summary report of all the
goals that employees are working on. We don't know who wrote the goals
. . . just that someone in the agency is working on that particular goal.
We use this information to plan our activities, so that we're giving you the
resources that will help you to achieve your goals! |
| Defensive Driving Course - Driving defensively impacts your wellness on several levels. Defensive driving impacts your occupational wellness: this course equips you with the skills to improve your safety as well as the safety of those you transport and those with whom you share the road. Your emotional wellness is improved when you learn to let go of aggressive driving habits and enjoy life in the safety bubble of the slow lane. Letting go of harmful habits (driving too fast, following too closely, road rage, etc.) is an excellent way of improving your physical wellness! Any time you learn something new, you're stimulating your intellectual wellness. Even though it's not one of our traditional dimensions of wellness, you can improve your financial wellness, by completing this course and presenting your course certificate to your auto insurance carrier in exchange for savings on your insurance premiums! That's a LOT of wellness for only $25 and 6 hours of your time! |

Safety Counts! If you participated in a fire drill or evacuation drill at your worksite, be sure to note it on your Wellness Activity Report. Workplace safety activities enhance your occupational wellness! |
