Optimal individual and community health and wellness through accessible, efficient, and coordinated health and human service delivery systems.
Assessment & Development - Gathers and reviews data related to health rankings, factors, and outcomes. Identifies available services, gaps in services, and facilitates planning to address identified gaps.
Care Coordination - Reviews and identifies effective models for care coordination. Develops centralized processes for coordination and analytics.
Education & Engagement - Develops and implements clear and consistent messaging across member organizations to promote available services, positive health behaviors, and educational materials for community members, business leaders, and elected officials.

The CNYCC is the Central New York Care Collaborative, a collaboration betweeen more than 300 (and growing daily) healthcare and community based service providers located throughout a ten county (Cayuga, Lewis, Madison, Oneida, Onondaga, and Oswego) region in Upstate Central New York.
