- WIC (Women, Infants, and Children) Program – a federal nutrition program that provides food, nutrition, counseling, and referrals to health care at WIC clinics located at 8 sites throughout the county.
- BFFNY (Breastfeeding, Chestfeeding, and Lactation Friendly New York) Program
Health Homes provides care management services to help make sure everyone involved in an individual’s care is working well together and sharing information that is important in supporting a person’s health. Health Home Care Managers help coordinate not just medical, mental health, and substance abuse services, but the social service needs of the individual as well. OPTIONS works with women and their families to improve the health of moms and babies throughout Oswego County by helping moms and dads to gain health care and connections to community resources. One on one education on women’s health and infant health are offered through home visits or by phone and text.
