Developmental Disabilities Services

Self Direction:

Self-Direction is the practice of empowering people with developmental disabilities to manage the supports and services they receive, determine who provides the supports, and how and where they are provided. In Self-Direction, the person with developmental disabilities chooses the mix of supports and services that work best for them, how and when they are provided, and the staff and/or organizations that provide them. This program is self-led, meaning participants take an active role in managing their services. The participant is responsible for submitting documentation and managing their own staff, with guidance and support from their FI and Brokers.

 At Oswego County Opportunities, Inc. we provide Fiscal Intermediary (FI) and Support Broker Services for Oswego County. Our in-house brokers work directly with our OCO, Inc. Fiscal Intermediary only.

 Eligibility:

· Participant must meet New York State Eligibility requirements / Medicaid Eligibility

· Participant must have an assigned Care Manager

· Participant must complete the Front Door process and qualify for OPWDD services.

· Participant lives in Oswego County
