Mandated Reporter Information
Mandated Reporters are professionals required by law to report suspected abuse and/or maltreatment of a child or vulnerable person.
A Vulnerable Person is a person who, due to a physical or cognitive disabilities, or the need for services or placement, is receiving care from a facility or provider. A vulnerable person can be an adult with developmental disabilities, with a mental health diagnosis, in treatment for chemical dependence or substance abuse, or a senior citizen in supervised residence or supportive care facility.
OCO Employees are a mandatory reporter while:
- Performing a job for OCO and
- they have reason to suspect abuse or maltreatment.
- CALL – You must call the state hotline if, while you are working for OCO, you have reason to suspect that a child or vulnerable person in our care is being abuse or maltreated.

If you believe that a person is in immediate danger, call 911 or your local police department.
- FILL OUT A REPORT – You must also fill out an OCO Incident Report form AND the required state report.
- For Child Abuse/Maltreatment fill out LDSS-2221-A – Mandated reporters must file, within 48 hours of an oral report, a signed, written report, Report of Suspected Child Abuse or Maltreatment (LDSS-2221A) (Link downloads a Word document)
